Welcome to North Palm College of Nursing - ASN Program Application
Congratulations on your decision to apply to North Palm College of Nursing! We're excited to help you achieve your educational goals.
We estimate it will take approximately 45-60 minutes to fill out the application. You need a complete application packet by the program deadline. Applicants must be 18 years old before or by the first day of the Program's start. The checklist on the next page will help you verify and ensure you completed ALL the admission requirements before sending your application to Admissions. If any box has been left unchecked, your application will not be suubmitted. This application may be used by degree seeking, certificate seeking and non-degree seeking applicants.
Please note that submission of an application does not guarantee admission to the program. All admission decisions are made by the Admissions Committee and are final. Any intentional misrepresentation or omission of information may result in denial of admission or dismissal from the program.
Important Note: Please make sure your name and date of birth are the same on your application for admission, all testing forms, transcripts/mark sheets, and other documents. Using special characters such as accent marks or non-English letters or symbols will cause issues with matching your application with important documents such as transcripts and test scores.
U.S. Permanent Residents Only: If you have Permanent Resident status in the U.S., please email a copy of your Resident Alien Card (P.R. Card) to the Admission Office at apply@northpalmcollege.com.
Colleges and universities are asked by many groups, including the federal government, accrediting associations, college guides, and newspapers, to describe the ethnic/racial backgrounds of their students and employees. In order to respond to these requests, we ask you to answer the following two questions.

North Palm College of Nursing takes the safety and security of its students, faculty, and staff very seriously. As part of the application process, we require you to provide information regarding your criminal history. Please carefully read and follow the instructions below:
1. Disclose any past criminal convictions, charges, or pending cases you have been involved in, including misdemeanors and felonies.
2. Be thorough and provide accurate details regarding the nature of the offense(s), dates, and jurisdictions involved.
3. Include any court-ordered probation, parole, or community service you have completed or are currently undergoing.
4. If applicable, provide information about any expungements or legal actions taken to clear your criminal record.
5. Please note that a criminal record does not automatically disqualify you from admission. However, all applications are reviewed on a case-by-case basis.
It is important to remember that providing accurate and honest information is essential. Falsifying or omitting details related to your criminal history may have serious consequences, including revocation of admission or enrollment.

Please provide accurate and complete information regarding your medical history in this section. It is essential for us to have a comprehensive understanding of your medical background to ensure your safety and well-being during your time at North Palm College of Nursing.

6. Ensure that you accurately disclose any infectious diseases or conditions that may pose a risk to yourself or others within the college community.

If you responded “Yes” to either of the questions, you are required to send the following items:
Please provide a letter from a licensed health practitioner, who is qualified by skill and training to address your condition, which explains the impact your condition may have on your ability to practice your profession with reasonable skill and safety, and stating either that you are safe to practice your profession without restriction or indicating what restrictions are necessary. If necessary, you may attach additional sheets. Documentation must be current within the last year.

If you fail to disclose the information requested in this section, your application may be denied.
Please list any honors, awards, or recognition you have received during high school. The information provided in this section may be considered for merit scholarship opportunities.
Please add your academic experiences below. As part of the application submission process, non-degree seeking applicants must upload unofficial transcripts, indicating an earned bachelor's degree/bachelor's equivalent or higher degree. Official transcripts must be sent to the Admissions office before students can enroll in future terms.
Make sure you include a transcript key/grading scale for each transcript and do not upload encrypted documents. Please do not upload transcript evaluations completed by third parties (e.g. ECE, WES, etc.). Upload copies of your transcripts only. Please email Admissions at apply@northpalmcollege.com with questions or for assistance.
Current or former NPC students: You are not required to upload unofficial transcripts or provide official NPC transcripts. We have access to your record via the student information system.
International applicants: Only upload copies of your transcripts/mark sheets with courses and grades, in English. At this time, diplomas, degree certificates, graduation certificates, etc. are not needed.
As an applicant to our ASN program, you must achieve a passing score of 60 or higher on the TEAS test. You are allowed three attempts per calendar year (January through December) with at least 30 days between attempts. If you do not achieve a passing score, you cannot retake the TEAS until the following calendar year.
Please note that passing the TEAS test does not guarantee admission. If you do not have a prior passing test score, you cannot retake the TEAS until the following calendar year. However, if you do not pass, we assess each case on an individual basis. Please contact our Enrollment Advisor for further instructions.
Upon acceptance into our ASN program, students must complete additional requirements for clinical participation, which involve additional costs. Please visit our website's Nursing page for information on Program Expenses, as some expenses and costs may be subject to changes.
Some health requirements include a physical exam certified by a licensed physician, proof of health insurance, a CPR certification (Basic Life Support for Health Care Providers and must follow the AHA guidelines), and up-to-date immunizations. Please note that North Palm College does not require the Covid-19 vaccine. However, the vaccine may be required by the hospitals and medical facilities where our nursing students are assigned to meet the clinical program requirements. If you have a medical or religious exemption, please contact us for further instructions.
In addition, students will need a background check and drug test, which will have an added cost. Please be advised that certain certifying agencies for health science professions require that individuals disclose their prior criminal history and arrest/conviction records if any. Students will go through a background check before the Program begins. Some agencies will not permit individuals with prior arrest/conviction records to become certified. Should you have a prior arrest/conviction record, you may wish to contact the certifying agency for your Program of choice to clarify whether your record will affect your certification or licensing eligibility. Some charges that affect admission and clinical participation, but are not limited to, are felony charges, incarceration, pending charges, and disqualifying misdemeanors. Note that the drug test will be administered unannounced. The drug screening will provide proof that the student is free from the use of any illegal drug and un-prescribed controlled substance. Medical marijuana is NOT permitted. Students taking any prescribed, controlled medication must provide documentation if requested. A student who fails to complete the required drug screening or receives positive results will be dismissed from the Program. If interested, the student is eligible to reapply for the next admission deadline.
After admission into the Program, before or by the Program Orientation, the Clinical Liaison Office will provide further information about the health documents required for clinical and instructions on completing these requirements.

I consent to North Palm College of Nursing (the College) taking one or more of the following actions upon discovery, at any time, of any such omission or misstatement of mine in this application: (1) Voiding of my admission to the College; (2) Voiding of my registration with the College; (3) Voiding of credit(s) for course work completed at the College; and (4) Distribution of information relating to such omissions and/or misstatements to other academic institutions, governmental agencies, and other third parties.
