NEXT-LEVEL UNDERGRADUATE REVENUE SHARE EDUCATION (NURSE) - Program Application

Congratulations on your decision to apply to the NURSE Program ! We're excited to help you achieve your educational goals.

We estimate it will take approximately 10 minutes to fill out the application. Applicants must be at least 18 years old. The checklist on the next page will help you verify and ensure you completed ALL the application requirements before submitting your application for review. If any box has been left unchecked, your application will not be submitted. This application may be used by degree seeking, certificate seeking and non-degree seeking applicants.

IMPORTANT:

1. Ensure that your name and date of birth are consistent across all application documents, including identification, income statements, and other supporting materials.
2. Avoid using special characters, accent marks, or non-English letters/symbols that may cause issues with matching your application with important documents.
3. If you are a U.S. Permanent Resident, provide a copy of your Resident Alien Card (P.R. Card) to the Underwriting Department.
4. Confirm that you meet the age requirement of 18 years or older.
5. Prepare necessary documentation, such as proof of income, identification, and address verification, for the application process.
6. Be aware that a credit check may be conducted as part of the application evaluation.
7. Review and acknowledge the terms and conditions of the NURSE agreement.
8. Express your interest in participating in the NURSE program and your understanding of the financial structure.
9. Provide accurate and up-to-date contact information for communication purposes.
10. Commit to fulfilling your obligations under the NURSE agreement, including making income-based payments as agreed upon.

Electronic Signatures & Information Consent

ELECTRONIC DOCUMENTS AND SIGNATURES CONSENT

NURSE OPPORTUNITY FUND, LLC., and/or any person or entity acting on behalf of or servicing your agreement for the aforementioned entities (collectively, "we" or "us" or "NOF") are required to provide You with certain written disclosures, documents, and information related to Your transactions with NOF.

Under the federal E-SIGN Act and related state laws, with Your consent, we can deliver this Required Information to You electronically and can use electronic signatures in connection with Your transactions with NOF. This Electronic Documents and Signatures Consent (the "Agreement") relates to Your use of NOF's services and Your NURSE agreement account ("NURSE Account"). Please read the terms of this Agreement prior to giving Your consent, and please maintain a copy of this Agreement for Your records.

By electronically agreeing to, and signing, this Agreement below, You consent and agree to proceed with the review and execution of all account agreements, disclosures, statements, and documents relating to Your NURSE Account in electronic format. This includes the use of electronic communications, electronic disclosures, electronic statements, electronic contracts, and electronic signatures (including "checkbox" style acknowledgments).

If You do not agree to proceed with: (a) the signing of the agreements electronically, (b) the review of disclosures and statements in electronic format, and (c) the receipt of electronic communications from NOF please decline to execute this Agreement. If You want to proceed with Your NURSE agreement application, You must consent to receiving the Required information electronically before we can provide it to You. Your consent will apply to all Your NURSE agreement applications and all other transactions with us. Your consent permits the general use of electronic records and electronic signatures in connection with all Your transactions with us.

Agreement to Conduct Business Electronically

By executing this Agreement, You are agreeing: (a) to the presentation of electronic documents, agreements, and disclosures, (b) that such documents are binding contracts, and (c) that You will accept and execute the applicable documents electronically. You acknowledge that all disclosures required by applicable law are being provided to You and may be provided electronically. You may also access and review electronic copies of all electronic documents on our website.

You further agree as follows:

• Your execution of this Agreement signifies Your intent to enter into a binding legal agreement, and that Your consent to conduct business electronically applies to all documents that NOF will provide to You or agreements that You will execute with NOF.
• All documents, including any modifications to such documents, signed using electronic means are as valid and enforceable as if You executed a paper copy using a "wet ink" handwritten signature.
• The authoritative copy of the electronically executed documents (the "Authoritative Copy") shall be the electronic copy that resides in NOF's document management system or that of its agents. If a paper version of the Authoritative Copy is created, then both the electronic version and paper version shall be considered originals and shall both be an Authoritative Copy.

Paper Copy of Required Information Available Upon Request - No Charge

You may print a copy of any Required Information from Your computer or emails. If You consent to receive the Required Information electronically, You can also request a paper copy of the Required Information by contacting us at (302) 262-1520. We will not charge You any fees for providing a paper copy of the Required Information. Your consent does not mean that NOF must provide the Required Information electronically—only that it may do so. We may, at our option, deliver Required Information on paper, if we choose to do so. We may also require that certain communications from You be delivered to us, on paper, at a specified address.

If You consent to receive the Required Information electronically, we will contact You at the email address You have provided to us. If You change Your email address, You must provide us with Your new email address either by logging into Your account and updating Your email address or by contacting us by email at nurse@nurseopportunityfund.com.

To access the Required Information being provided to You electronically, You must have the following, which may be updated from time to time:

• A personal computer or mobile device with internet access and one of the following web browsers, or a more recent version of these web browsers, installed:
  • Google Chrome, latest version
  • Internet Explorer, latest version
  • Microsoft Edge, latest version
  • Safari versions 10 and above
  • Mozilla Firefox, latest version
  • iOS 10 & 11
  • Android 5+
• The ability to receive email that contains hyperlinks to websites in order for us to deliver information to You.
• Software which permits You to receive, print and access Portable Document Format or "PDF" files, such as Adobe Acrobat Reader®. If You do not have such software, click here to download a free version of Adobe Acrobat Reader®.
• Sufficient electronic storage capacity on Your computer's hard drive or other data storage unit or the ability to print PDF files from Your computer or tablet device.

By checking the box and proceeding, I am electronically signing this Agreement. I agree to the terms of this Agreement, effective as of today's date, and confirm that I have computer software and hardware that meets the requirements above. I CONSENT TO (I) RECEIVING REQUIRED INFORMATION ELECTRONICALLY, AND (II) THE USE OF ELECTRONIC RECORDS AND SIGNATURES IN CONNECTION WITH MY NURSE AGREEMENT APPLICATION AND ALL OTHER TRANSACTION(S) WITH NURSE OPPORTUNITY FUND. NURSE OPPORTUNITY FUND IN PLACE OF WRITTEN DOCUMENTS AND HANDWRITTEN SIGNATURES. Once You give Your consent, You can view Your documents online or by reviewing Your emails. YOU ALSO ACKNOWLEDGE THAT YOUR CONSENT TO ELECTRONIC DISCLOSURES IS REQUIRED TO RECEIVE SERVICES FROM US OVER THE INTERNET.

TELEPHONE CONSUMER PROTECTION ACT (TCPA) CONSENT

By clicking "I have read and agree to the TCPA Consent" and continuing with the application process, you agree to receive communications from NURSE OPPORTUNITY FUND, LLC., and/or any person or entity acting on behalf of or servicing your agreement for the aforementioned entities (collectively, "we" or "us" or "NOF") to address any technical problems associated with Student's NURSE agreement application, or to send Student notices regarding Student's NURSE agreement approval, payment reminders, or collection efforts, or other funding opportunities. Communication may be sent to any telephone number, email address, or other contact information that you provide in your application or in the future via any current or future means of communication, including but not limited to: automated telephone dialing equipment, as defined by law, the FTC, or the FCC; artificial or pre-recorded voice messages; telephone or other electronic facsimile machine; SMS or text messages; and email directed to you at a mobile telephone service, or email otherwise directed to you.

You acknowledge that such communications are informational communications and are not made for the purpose of telemarketing. NOF (and/or any person or entity acting on NOF's behalf or servicing your agreement for NOF) may use such means of communication even if you will incur costs to receive such telephone messages, telephone calls, SMS or text messages, emails, or other means.

If you do not want to receive calls (including SMS text messages) about your account you can unsubscribe through one of the following:

NURSE Cost Example

The total amount you will pay for this NURSE Agreement will vary based on your future earnings. This example provides estimates based upon different income scenarios. THIS EXAMPLE ASSUMES (i) YOU WILL RECEIVE $40,000 IN FUNDING; (ii) YOU WILL BE ENROLLED IN SCHOOL 18 MONTHS FROM YOUR FUNDING DATE, AND (iii) YOU WILL HAVE A 6-MONTH TRANSITION PERIOD.

Federal Loan Alternatives

You may qualify for Federal education loans. For additional information, contact your school's financial aid office or the Department of Education at: http://www.studentaid.gov.

Next Steps

1. Find Out About Other Loan Options. Some schools have school-specific financing benefits and terms not detailed in this form. Contact your school's financial aid office or visit the Department of Education's website at: http://www.studentaid.gov for more information about other loans.
2. To Apply for this NURSE Agreement, Complete the Application and Self-Certification Form. You may get this certification form from your school's financial aid office or contact us for assistance. If you are approved for this NURSE Agreement, the terms will be available for 30 days (terms will not change during this period, except as permitted by law).

NURSE Agreement

A NURSE Agreement is your obligation to make payments which are indexed to your income. A NURSE Agreement is not an assignment of your income, rather we will use your income to determine the amount you owe to us. Throughout the term of your NURSE Agreement, your NURSE percentage will not change. Thus, if your income increases, your payments will increase. If your income decreases, so do your payments. The amount you pay to us may be more or less than the amount you receive.

Eligibility Criteria

• Be in good academic standing, meeting satisfactory academic progress
• Have no significant adverse credit events
• Be a U.S. citizen, permanent resident, or DREAMer with a current DACA authorization
• Be at least 18 years old at the time you sign the agreement

Monthly Payments

• If you are still in school, you will begin making payments on the first day of the month following six months after you graduate, withdraw from school, or are no longer considered to be enrolled. If you are no longer in school and will use funds from this NURSE Agreement to cancel and replace one or more prior NURSE Agreements with NURSE OPPORTUNITY FUND and one or more of those NURSE Agreements is in a Transition Period, your payment term will begin on the first day of the month following the end of the Transition Period for the NURSE Agreement with the most time remaining in its Transition Period. In all other cases, your payment term will begin on the first day of the month occurring after one whole month has passed from the effective date of your NURSE Agreement. We will calculate your first monthly payments using a verifiable source of your income acceptable to us.
• Each year we will reconcile over- or under-payments made in prior year using your year-end IRS tax documentation approved by us. You must reimburse us for any under-payments and we will credit your account for any over-payments.
• You will owe nothing in months where your payments are below the Lower Income Cutoff—an amount that is set in your NURSE Agreement.

What Happens if You do not Provide us with Required Information?

You will have an obligation to provide us with new tax documentation every year. If you do not provide us with that documentation by the specified due date, you will be charged a fee of $20.00 and be considered in breach of the agreement. Upon providing us with your tax documentation, we will adjust your payments accordingly and reconcile any over- or under-payments made during this time.

Payment Relief Pauses and Early Completion

• At your discretion, you can take up to 3 months of Payment Relief Pauses (temporary postponement of payments), continuously or separately, during the payment term. During a Payment Relief Pause, you will not be required to make payments. Months you use payment relief pauses will not be credited toward the Payment Window.
• You may terminate your NURSE Agreement at any time by making a payment equal to the Early Completion Amount.

Early Completion Clause:

Your payment obligation ends early when your payments (including any amounts paid beyond amounts that are due) reach the Early Completion Amount (your Loan Amount calculated at 17.5% annually), and you've paid any outstanding payments and fees.

Borrowers in the Military

Servicemembers Civil Relief Act ("SCRA"). If you are not currently an active duty servicemember and you either become an active duty servicemember or are called to active duty from a non-active duty status, please contact us immediately, you may have additional rights under the SCRA.

Bankruptcy

For private education loans, federal law limits your ability to discharge these loans in bankruptcy. NURSE OPPORTUNITY FUND does not believe this standard should apply to your agreement with us. If you file for bankruptcy, you may still be required to pay back a "Private Education Loan." In the event that you file for bankruptcy, NURSE OPPORTUNITY FUND will not assert that this agreement is subject to restrictions on discharge in bankruptcy that apply to Private Education Loans. If the bankruptcy court treats this agreement as a Private Education Loan and requires you to show "undue hardship" to discharge the obligation, and you otherwise have satisfied the requirements to obtain a discharge in bankruptcy, NURSE OPPORTUNITY FUND will take all steps necessary to abandon any claims NURSE OPPORTUNITY FUND make for future payments under this Agreement. This does not mean that NURSE OPPORTUNITY FUND we will waive any rights to claim that you should have paid any amounts to us before the date you filed for bankruptcy.

What are the ranges of terms?

Your NURSE Obligation Your NURSE Obligation Ends When You Meet One of These Three Finish Points:

The loan amount that has been disbursed to you, on your behalf, and/or used to cancel prior NURSE Agreements.
The percentage that is used to calculate your monthly payment. We will multiply the percentage of income by your gross monthly income to determine your monthly payment amounts (when you have income above the Lower Income Cutoff).
You are required to make payments when your gross earned income (expressed as an annual amount) is at or above this amount.
When your gross earned income is above this amount (expressed as an annual amount), your income-determined payments will not continue increasing.
The maximum number of monthly income-determined payments (when your income is above the Lower Income Cutoff) needed to satisfy the NURSE obligation.
If, after each monthly payment, we calculate that you would have paid off a loan with a 5.5% interest rate and with the same amount, same disbursement, same payments, and same timing of payments, your obligation is immediately done under this Early Completion Clause.
Your obligation ends 240 months after you enter the payment window, excluding any months where you use a Payment Relief Pause. Your payment window begins after you leave school and after your Transition Period has ended.

How long will my obligation last?

Minimum Term (<1 month) Maximum Term (252 months)

The shortest time your obligation could last. This case occurs if you trigger the Early Completion Clause in your first month out of school.
The longest time your obligation could last. This does not include the period of time when you are in school or any time included in the Transition Period. This case occurs if you have at least 120 months of zero payments (months where your income is below the Lower Income Cutoff) and you use all 3 of the Payment Relief Pause months available to you.

Your Obligations While in School

• Update your current contact information any time your contact information changes
• Update your enrollment status and whether you are graduating or plan to take a leave of absence or leave school
• Complete & send any documents we request

Your Obligations After You Leave School

• Make your NURSE Agreement payments
• Tell us if there is any change to information you have previously provided to us
• Inform us of any change in employment status
• Let us know if there is any increase or decrease in your income

Who we are

Who is providing this notice? NURSE OPPORTUNITY FUND, LLC is providing this privacy policy and it applies to all NURSE agreements made by the company and all products and services offered in connection with such NURSE agreements.

What we do

How does NURSE OPPORTUNITY FUND protect your personal information? To protect your personal information from unauthorized access and use, we use security measures that comply with industry standards. These measures include computer safeguards and secured files and buildings.

How does NURSE OPPORTUNITY FUND collect my personal information? We collect your personal information, for example, when you:

• Apply for a NURSE agreement
• Give us your income information
• Give us your contact information
• Tell us where to send your money
• Provide account information
• Provide employment information
• Respond to a survey or research questions

We also collect your personal information from others, such as credit bureaus, affiliates, or other companies.

Why can't I limit all sharing? Federal law gives you the right to limit some but not all sharing related to:

• Affiliates' everyday business purposes - information about your creditworthiness
• Affiliates from using your information to market to you
• Non-affiliates to market to you

State laws and individual companies may give you more rights to limit sharing.

Definitions

Affiliates: Companies related by common ownership or control. They can be financial or nonfinancial companies.

Our affiliates include: NURSE OPPORTUNITY FUND.

Nonaffiliates: Companies not related by common ownership or control. They can be financial and nonfinancial companies. Nonaffiliates we share with include collection agencies, servicers, consumer reporting agencies, contract buyers, investors, database providers, and others.

Joint Marketing: Formal agreement between nonaffiliated financial companies that together market financial products or services to you. We do not share your information for joint marketing purposes.

Credit History Authorization — FL Program

NURSE OPPORTUNITY FUND (NOF) does not use your credit scores to determine funding eligibility; however, NOF does pull credit to check for other debt and adverse events, such as bankruptcy.

NOF will pull your credit after you have an informational meeting with a NOF team member.

CREDIT HISTORY AUTHORIZATION

I hereby authorize NURSE OPPORTUNITY FUND, LLC, and its agents, to request and obtain:

1. Information about me from various federal, state, and other agencies
2. Information about me maintained in public and private record sources
3. Credit histories maintained by various credit bureaus or ratings agencies
4. Information on previous contractual engagements
5. Criminal background information, if applicable, as well as other past experiences, all as determined by NOF

I acknowledge and agree that NOF is permitted to use this information in connection with validating my identity and its evaluation of my application for a NURSE agreement from NOF. I am expressly providing written instructions (as provided in the Fair Credit Reporting Act) to NOF authorizing NOF to obtain information from your personal credit profile or other information from one or more consumer reporting agencies, such as Equifax®, Experian®, and TransUnion®.

I agree not to assert any claims or causes of action against NOF, its agents, its employees, or any individuals contacted by NOF, arising out of this due diligence investigation. NOF agrees to use any information collected herein for investigatory, underwriting, modeling, analysis, and due diligence purposes (and any related purposes). This information may be shared with a third-party, to administer the sale of my NURSE agreement, the associated payments receivable, and/or payment dependent notes.

I authorize NOF to obtain consumer reports each time I request a new NURSE agreement, during the processing or closing of your NURSE agreement (if approved), or at various times during the term of my NURSE agreement (if approved) in connection with the servicing, monitoring, collection, or enforcement of the NURSE agreement.

By checking the box to indicate my acceptance of this agreement and proceeding with the application, I will electronically sign this authorization. I certify and agree to the terms contained in this authorization and also acknowledge receipt of a copy of this authorization, which was provided to me electronically, and in paper form, if I so requested it.
